Comprehensive assessment of the senior's physical, emotional, cognitive, social, safety, financial, and environmental needs — followed by a detailed care plan and resource recommendations tailored to the senior and/or their family.
Regular home visits to reassess and update the plan of care as needs evolve. Ongoing phone availability for questions and concerns from both the senior and designated family members or caregivers.
Assistance maintaining accurate medication lists, reminders, pharmacy coordination for refills and pickups, and ensuring compliance and safety with prescribed treatments.
Scheduling and navigating medical appointments, ensuring the necessary referrals are in place prior to the visits, accompanying seniors to their appointments, advocating during the visits, and providing feedback to designated family members and caregivers.
Ongoing home safety and risk assessments to help seniors remain safely at home as long as possible. If additional support becomes necessary, we assist seniors and/or their families in securing the right level of care across all levels of support:
Encouraging meaningful social interaction and reducing isolation through personalized activities. We also help seniors use technology to connect with loved ones and improve safety and communication.
Support with advance directives, living wills, healthcare surrogates, and future care planning — ensuring wishes are clearly communicated and a "Plan B" is always in place. We also assist in preparing for safe discharge planning after a hospitalization or rehab visit.
Guidance with Medicaid Long-Term Care applications and trusted referrals to eldercare attorneys for trusts, estate planning, asset protection, and Medicaid planning strategies.
Assistance arranging the most appropriate and safest transportation options for medical appointments and personal needs.
Compassionate, holistic, non-medical support navigating the emotional and practical complexities of the end-of-life journey — for individuals, families, and friends alike.
We serve as a compassionate advocate between the senior, their family and loved ones, and the medical and hospice team — ensuring the senior's voice remains central throughout. We work to ensure that the senior's wishes are clearly communicated, respected, and honored at every stage of the end of life journey.

We visit the senior in their home for a comprehensive assessment covering physical, emotional, cognitive, social, safety, financial, and environmental needs. This thorough evaluation forms the foundation of everything that follows.
A follow-up visit presents detailed feedback, recommendations, available resources, and a fully personalized care plan. Every plan is created specifically for your loved one — never generic, never one-size-fits-all.
We walk alongside you through regular home visits, phone support, care coordination, and crisis management. As needs evolve, the care plan evolves with them. You're never navigating alone.
Service Contracts
Service contracts are available in 6-month or 12-month terms. The level is determined by the number of monthly in-home visits required. 12-month contracts receive the 13th month free.
