Meet the Founder of
Navigating The Sunsets Together

Karen Grobler was born and raised in England — when her family moved to South Africa she felt the stress of leaving her grandmother far away. It troubled her: who would be there to provide hands-on assistance as she grew older? Would she get the support she needed in her 90s when she was trying to live independently but was becoming too frail and tired to manage home alone? These were the days before modern technology which now eases the burden of trying to stay connected.

Once in South Africa, Karen completed her training as a registered nurse and worked in a variety of fields including hospitals, doctor offices, and on the road conducting healthcare-related home visits.

Initial In-Home Assessment & Care Planning

Comprehensive assessment of the senior's physical, emotional, cognitive, social, safety, financial, and environmental needs — followed by a detailed care plan and resource recommendations tailored to the senior and/or their family.

Ongoing Assessments & Support

Regular home visits to reassess and update the plan of care as needs evolve. Ongoing phone availability for questions and concerns from both the senior and designated family members or caregivers.

Medication Management Support

Assistance maintaining accurate medication lists, reminders, pharmacy coordination for refills and pickups, and ensuring compliance and safety with prescribed treatments.

Medical Appointment Coordination & Advocacy

Scheduling and navigating medical appointments, ensuring the necessary referrals are in place prior to the visits, accompanying seniors to their appointments, advocating during the visits, and providing feedback to designated family members and caregivers.

Ongoing Home Safety & Safe Aging in Place

Ongoing home safety and risk assessments to help seniors remain safely at home as long as possible. If additional support becomes necessary, we assist seniors and/or their families in securing the right level of care across all levels of support:

We begin with a warm, no-pressure conversation — by phone or in person — to understand your situation, concerns, and what you're hoping to find. There's no obligation. We simply listen.

In-Home Assessment

We visit the senior in their home for a comprehensive assessment covering physical, emotional, cognitive, social, safety, financial, and environmental needs. This thorough evaluation forms the foundation of everything that follows.

Personalized Care Plan

A follow-up visit presents detailed feedback, recommendations, available resources, and a fully personalized care plan. Every plan is created specifically for your loved one — never generic, never one-size-fits-all.

Ongoing Partnership

We walk alongside you through regular home visits, phone support, care coordination, and crisis management. As needs evolve, the care plan evolves with them. You're never navigating alone.

Service contracts are available in 6-month or 12-month terms. The level is determined by the number of monthly in-home visits required. 12-month contracts receive the 13th month free.

An Eldercare Care Manager is a trained professional — often with a nursing or social work background — who helps seniors and their families navigate the complexities of aging. This includes healthcare coordination, care planning, advocacy, crisis management, and connecting families with the right resources. An End of Life Care Manager provides compassionate, non-medical support specifically during the final stages of life.
A home health aide provides hands-on physical care. Hospice provides medical and comfort care for those in the final months of life. Navigating The Sunsets Together fills the critical gap in between — we provide coordination, advocacy, planning, emotional support, and navigation of the entire system, working alongside your existing care team to ensure nothing falls through the cracks.
We serve most of Palm Beach County, including: Atlantis, Boca Raton, Boynton Beach, Briny Breezes, Delray Beach, Deerfield Beach, Village of Golf, Greenacres, Gulf Stream, Haverhill, Hypoluxo, Lake Worth, Lantana, Ocean Ridge, Palm Beach, and West Palm Beach. We also support families with loved ones who live in other states or countries, helping to bridge distances through technology and coordination.
This is one of the most common challenges families face, and Karen is experienced in navigating it with sensitivity. The first visit is always low-pressure and focused on listening to the senior — not imposing solutions. Building trust takes time, and we approach it at whatever pace feels right for your loved one.
Our services are provided under private-pay contracts and are not typically covered by Medicare or Medicaid. However, long-term care insurance policies sometimes include care management benefits. We are happy to help you explore this. We'll walk you through the contract structure and pricing during your initial consultation.
The sooner the better — but there is no wrong time. Many families reach out following a health scare, hospitalization, or when they notice a loved one struggling alone. Others come to us proactively to plan ahead. Whatever your situation, we welcome the conversation and will always meet you where you are.
